1. The three certificates that are needed need to be uploaded: Concussion in Sports, Sudden Cardiac Arrest, & Heat Illness Prevention
2. Driver’s License from parent must be uploaded – this is only to verify signatures.
3. Insurance – IGNORE THIS. Not required for Band and Color Guard members
4. No Physical is required for Band Members – ignore this. Color Guard must complete a physical on the proper EL2 form.

This is mandatory for anyone who will be around the students at any point this year. You do not need fingerprinting. Level 1 approval is sufficient. If you have done this in the past you may not need to do it again. Log-in to see your status and expiration.
